Please help me get rid of this annoying screen

Every single time someone calls me I have this screen pop up. And when call ends it does this. ![image][1]![image][2]

Try this!
Remove Alex Fraser from your contacts.
Clear the cashed data on your contacts along with your recent call log.
Then re-enter Alec fraser into your contacts!
And have Alec do the same on his phone!
