Situation:

I am designing an app that will streamline a certain functionality of an other app. It is almost like a plug in.

Question:

Can I include reference to this other app in the keywords, description, and content of my app? (i.e. name, logo, icon, etc.)

Otherwise, how will people know what my app is for?

You're going to have to be more specific. BUT my guess is that the company doesn't allow unauthorized plug-ins for their app.

It sounds like whatever you do, you're going to be at risk of getting your app removed. I'd contact the dev of the other app.
