Hi all. I was told that you need to get a document to demonstrate at the hotel or any other rental property that you require a service animal to be present. Can you help with that and tell me where I can get one?

You can fill out this application at 111 and I will explain to you what you need to do. If you want them to complete the documentation for the request instead of you, you will need to provide the following information:
- Your name.
- Address.
- Phone number.
Information about your service animal (type and breed, its name, what services it provides to you, etc.)
You may then have your personal housing application processed online or mailed to your address (depends on the option you choose). If there are no problems at the post office, you should receive it in about three to five business days.
